For Children entering 5th & 6th Grade
Once upon a time . . . in a far away land called Adventure Mountain, an awesome jungle camp was created for your child! It is an adventure-filled experience like no other!
As your child walks up the path to Adventure Mountain, he or she will instantly face the challenge to live life "to the max" while purposefully exploring this very BIG world. Around every tree and safari tent your child will enter into a deeper understanding of God's love, demonstrated through His Son Jesus Christ and our well-trained staff!
Adventure Mountain is a safe, fun environment for your 5th-6th graders to pursue a better understanding of life with Jesus. Through sound bibilical teaching and interactive games we aim to empower your child to pursue a life where Jesus is King.

Please download the attached Forest Home Youth Registration & Medical Consent Form, fill it out and mail it to:

There is a $100 deposit to reserve your spot, then you may make up to three additional payments, ending by June 9, 2013.
A NOTE ABOUT REGISTRATION:
Please be aware that you must fill in every blank or it will give you an error message (Example: If your child has only one roommate preference, please enter none in the other space).
